Biographical Notes:

Flying  an Oxford 1, from RAF Halfpenny Green in Staffordshire, England, with instructor Flt Lt Worswick, they encountered snow at 1000 ft and while climbing to 2800 ft both engines cut out due to carburettor icing. Attempting to land in zero visibility, the Oxford hit trees and crashed at Wassel Wood, near Kidderminster, Worcestershire. His remains were cremated at Harrogate, Yorkshire and returned to New Zealand by his brother. It was later revealed that pre-heater muffs had been ordered for the carburettors, but not delivered.
